For a college recommendation letter, two forms must be completed by the parent and the student. The completed forms should be emailed to the student's counselor. These forms need to be submitted at least 3 weeks before the first college deadline.

Some teachers require students to complete this form before the teacher will write a recommendation letter. Check with the teacher to see if he/she would like you to complete this form.

This form is for CURRENT WESTLAKE SENIORS to request their transcript to be mailed to colleges or scholarship Programs. Other Westlake students and graduates should use the RECORDS REQUEST form which is under the "Counseling & Enrollment" tab on the Westlake website.
